10-Step Instructions for filling out your time sheet:

  1. Employee Name (Print your name here)
  2. Company Name (The Company at which you working goes here)
  3. Service Name (i.e. Exchange Migration Services or Help Desk Services)
  4. For each day you work, complete the start time, finish time, subtotal, less lunch, and total hours. Please use .25, .5, and .75 in place of 1/4, 1/2 and 3/4 for your hours.

    Example:
    • Start time
    • Finish time
    • Subtotal hours
    • Less time taken for lunch
    • Total hours worked

  5. Indicate whether you want your check mailed or directly deposited in your checking account. If you have not signed up for direct deposit please call us at 978.620.1800.

  6. Enter the week ending date (Friday’s date for the week in which you work), your regular hours total, and your total for overtime (anything over 40 hours).

  7. You must SIGN in the section entitled employee signature.

  8. The supervisor to whom you report or person authorized to approve your hours MUST sign the section entitled Company Verification & Signature.

  9. When you have finished filling out the time sheet as outlined above, give a photocopy to your supervisor, fax a copy to our office 978.349.6099, and keep the original.

  10. Time sheet must be faxed to us by midnight Friday of the week you worked, or your check will be delayed until the following pay period. Pay periods are on the 15th and the last day of each month. It is YOUR responsibility to produce a time sheet for us.
